Thanks for providing some information about yourself. All such information
will be kept strictly confidential. First, we're going to ask you for some
basic information, and then some details that you might be asked if you were
called for jury duty.
Basic contact information
We don’t really need your full name or address, but it will help to have some
name -- your first name or a nickname or screen name -- you want to be called
when there is a jury deliberation. It must be unique, so we may ask you to resubmit
it if we find a duplicate already on our system. We will also need
a password so we have a confirmation that you are the right person. If you have
already signed up with PayPal, please use the same e-mail address you used with
them. (If you change your PayPal e-mail address, let us know.) We don’t need
your street address, but we do need city, county and state. Many lawyers are
going to ask for jurors from their county.
The first item is a referring number from the person who referred
you. That number insures that the person who referred you gets credit. If
there is a number already there, it came with the link you clicked on and
is correct. If you don't see a referring number in the box, but you know you
were referred by another juror, ask him/her for the referral number. You can
email it to us later if you like. If you weren't referred by anyone, leave
